Chronic Disease Management

Chronic Disease Management (formally the Enhanced Primary Care Program) is a federal government initiative through Medicare. There is one Medicare Benefits Schedule (MBS) component. Let's Play Developmental Therapy Pty Ltd can provide Occupational Therapy services eligible for this rebate.

10958 - key features:

• Child eligibility - referred by a General Practitioners (GP) for having a chronic condition and complex care needs. Can be accessed by children diagnosed with an Autism Spectrum Disorder (ASD), Asperger's syndrome, PDD-NOS, and Sensory-Motor Disorder
• Rebates - up to 5 therapy sessions total per calendar year
